What to expect
Being well-prepared for your appointment will ensure that the physician has all of the needed information to provide the best possible care for you.
Please arrive 30 minutes prior to scheduled appointment time to complete registration.
We require the following items to be brought with you to your appointment:
- Identification card (drivers license)
- Current medical insurance card(s)
- A written or typed list of all medications, including strength and dose
- Referral form(s) if required by your insurance company
- Co-pay, deductible, and co-insurance, required by your insurance company
- Completed registration forms
